4. How to Fill Out Team Member Change Form
Enter employee number into the first field
Legal Last Name - Enter your legal last name
Legal First Name - Enter your legal first name
Location - Select 129 DFA Wpg Service Center
Email Address - Enter your personal email address
Name Change - Choose Yes or No if you would like your name changed on file
Date of Birth Correction - Choose Yes or No if your birth date is incorrect on file
Social Insurance / Work Permit Info - Choose Yes or No if your social insurance number or your work permit information has changed
Address Change - Choose Yes or No if your address, email address, or phone number has changed
Primary Province - Select your Province from the list
Emergency Contact Change - Choose Yes or No if your emergency contact has changed
Banking Change - Choose Yes or No if you would like to update your banking details
Certify / Upload - If you are making a change to your banking or social insurance number/work permit, you need to include a copy to support the change .
